I am excited to participate in your event.

For most of my home and party visits, I have a fairly detailed form to complete, helping the family/host to create the perfect environment for the perfect visit.

In the case of events, I only have a few basic questions. But I ask that you be very specific about what exactly you want your event to look like.

Please complete this form no later than a week prior to the event. And contact me directly if you have any questions.

Additional Questions for me

Upon submission, we should be ready for the event! By now you should have signed a contract, made a deposit and final payment (due one month out from visit), and completed this form. I will contact you with answers or if I have any questions or need clarification, and I ask that you please do the same.
